*Graduates of Excelsior College and other non-traditional programs should contact the Board before applying for license
Temporary permits are issued while other required docs are in process of receiving a permanent license. Additional necessary steps to receive a permanent license MUST also be initiated.
Inactive status 2 years or more – must provide proof of current licensure in another state with employment verification of at least 520 work hours within 2 years. Evidence of status form required if lapsed on or before November 1, 2007.
