As a Fastaff travel nurse you will be required to complete a variety of applicant and payroll forms in order to be placed on assignment with us.  For your convenience, we suggest you download and print the forms listed below and either fax or email them back to us.

All the forms are in Adobe Acrobat (PDF) format and can be downloaded and saved for future reference. If you do not already have Acrobat Reader installed on your computer, please download it for free on Adobe’s website.
